Speech Language Pathologist- P/D- (North Star)

Location: Serving South and East LA County
Job Summary: The practice of speech-language pathology shall include assessment, treatment, planning, implementation, education, communication and demonstration of techniques addressing speech-language, cognitive and swallowing disorders to meet the needs of patients and their families. Requires minimum supervision.

Essential Functions:
? Completes all assessments/evaluations, including comprehensive assessment of speech-language, cognitive and swallowing deficits at specific time-points as defined by regulation and agency policy, accurately and timely. May complete OASIS if indicated.
? Develops a plan of care in collaboration with the patient, family/caregiver and physician orders including frequency and duration of speech pathology visits, specific interventions, appropriate goals, other disciplines needed and discharge plan.
? Documents the assessments/evaluations and plan of care in the HER, documents and synchronizes by 10:00AM the following day.
? Performs and documents speech pathology interventions including patient education according to the plan of care and adapts these as patients? needs change.
? Practices infection control and universal precautions for self and others in patient care activities.
? Advocate for quality patient outcomes communicating with authorization nurses and community resource agencies timely.
? Participates in patient care conferences, and team meetings as indicated
? Maintains patient rights and dignity.
? Annually completes compliance education and understands applicable policies.
? Meets productivity standards for the organization
Additional Responsibilities:
? Performs other duties as assigned or requested.
? Possesses knowledge of Medicare/Medical and insurance standards to ensure
reimbursement for all services rendered.
? Adheres to California Licensing Regulations for Home Health and performs duties within the Speech
Language Pathologist scope of practice.
? Conforms to all applicable Agency policies and procedures.
? Participates actively in continuing education and in-services.
? Assumes responsibility for reporting incidents and complaints according to Agency policy.

? Is knowledgeable of, observes and complies with Home Health Network Codes of Safe Practice, safety policies and emergency procedures.
Minimum Position Qualifications:
? Education: Completes necessary education to attain Speech Therapy license in the state of California.
? License / Certification: California licensure by the Speech-Language Pathology and Audiology board; Basic Life Support certification; California driver's license and proof of current auto liability insurance.
Preferred Position Qualifications:
? One year clinical experience as a Speech Pathologist.
? Prior Home Health experience as a Speech Pathologist.
Environmental Conditions:
? Knowledge of proper procedures for handling exposure to communicable diseases and blood borne pathogens.
? Knowledge of the proper technique to handle exposure to hazardous material.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
